If you are under 40 years of age and applying for a private car or two-wheeler license (non-transport), you only need to submit the Form 1 Self-Declaration. You do **not** need a doctor's Form 1A certificate. Form 1A is only required if you are age 40+ or applying for a transport license.
To get an online medical certificate, visit a registered medical practitioner listed on the **Sarathi Parivahan RTO doctor database**. The doctor will perform standard vision and physical tests, and log onto their dedicated portal using their credentials to upload your Form 1A directly to your DL application number.
A Form 1A Medical Certificate is legally valid for **one year** from the date of issue. Your driving licence application or renewal must be submitted to the RTO within this one-year period.
